Young Dental Essay Contest

CONTEST OUTLINE: Young Dental (the “Sponsor”) is holding an essay contest (the “Contest”).  The winner will receive a trip to the RDH Under One Roof meeting 2012 (the “Prize”).

DURATION:  The Contest begins at 12:00am CST on Monday 9/1/2011.  All entries must be received by 11:59pm CST on 6/1/2012.  This timeframe will be known as the “Contest Duration”

ELIGIBILITY REQUIREMENTS:  The Contest is open to legal residents who are practicing dentists, dental hygienists, dental assistants, or students in a dentistry-related course of study.  Participants must be at least 18 years of age, reside in the 50 United States or District of Columbia.  Employees of Young Dental, its parent company, subsidiaries, partners, or other constituents of the Contest are not eligible to participate.

HOW TO ENTER:  Essays, as part of an entry into the Contest, must be 300 words or less and respond to the following prompt:

“Caring for You and Your Patients has long been a goal for Young Dental. The saying carries different meanings to many people. For us, its a testament to our belief that the use of Young products makes it easier for you to help your patients live a better life through a healthier mouth. We truly hope you feel the same way, so wed like to hear your thoughts. How do you go above-and-beyond in caring for your patients and community?”

Entries in the Contest can by submitted by visiting www.youngdental.com/essay, completing the form, and submitting.  Entries can also be submitted in writing by mailing your name, age, gender, address, email address, phone number, practice name, and essay to “Young Dental / attn: Essay Contest / 2260 Wendt St / Algonquin, IL  60102”.  Only items postmarked before the deadline will be accepted.  Limit (1) entry per person during Contest Duration.

CONTEST PROCEDURE:  After the conclusion of the Contest Duration, all eligible entries will be judged by a panel (the “Panel”) of Young Dental employees and key opinion leaders.  The actual members of the panel is to be determined at a later date.  Essays will qualitatively scored based on content, not writing ability.

PRIZE AND NOTIFICATION:  The author of the essay determined to be the winner by the Panel (the “Winner”) will be awarded the prize (the “Prize”) of a trip to the RDH Under One Roof Meeting in 2012 (the “Meeting”).  The Prize includes roundtrip economy airfare for (1) person on a reputable airline from a major airport near where the Winner resides to an airport near the Meeting, full registration to the Meeting, 3 nights hotel accommodation where the Meeting takes place, a 3-day per diem allowance for discretionary spending of $30 to be paid in full to the Winner in advance of the Meeting, and an opportunity to be present and the Sponsor’s exhibit booth during the Meeting (if the Winner desires).  All expenses and costs not expressly listed above are solely the Winner’s responsibilities. Any and all items including the Prize that the Winner has already purchased are eligible for reimbursement with proof of purchase. The estimated value of the Prize is $2000.  Odds of winning the Prize are based on the number of entries.  No substitution or transfer by the Winner of the Prize is permitted

The Winner will be notified by the phone number, email address, and mailing address provided in the winning entry.  The Winner will be required to sign and return an Affidavit of Eligibility and a Liability of Publicity Release to be returned within 15 days of notification at risk of forfeiture of Prize.

LIMITATION OF LIABILITY:  All essays, once submitted, will become property of the Sponsor and will not be returned. Winner agrees that Sponsor may use Winner’s name, photo/likeness, and contents of essay for advertising and promotional purposes without further compensation.  The sponsor, it’s promotional and advertising agencies, and all respective officers directors, employees, representatives and agents shall have no liability and shall be held harmless by winner for any damage, loss or liability to person or property, due in whole or part, directly or indirectly, by reason of the acceptance, possession, use or misuse of prize or participation in the Contest Any and all disputes, claims and causes of action arising out of or connected with the Contest, or any prizes awarded, shall be resolved individually, without resort to any form of class action, and exclusively by arbitration. Any and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, including costs associated with entering the Contest, but in no event attorney's fees.

Sponsor reserves the right, in its sole discretion, to cancel or suspend part or all of this Contest should virus, bugs, non-authorized human intervention or other causes beyond their reasonable control corrupt or impair the administration, security, fairness or proper play of the Contest. In such event, Sponsor may award the Grand Prize in a random drawing from all eligible entries received up to the date of cancellation or suspension. Sponsor and its promotion and advertising agencies are not responsible for technical, hardware, software or telephone failures of any kind, lost or unavailable network connections, fraud, incomplete, garbled or delayed computer transmissions, whether caused by the Sponsor, users or by any of the equipment or programming associated with or utilized in the promotion or by any technical or human error which may occur in the processing of submissions which may damage a user's system or limit a participant's ability to participate in the promotion. Sponsor may prohibit an entrant from participating in the Contest or winning a prize if, in its sole discretion, it determines that said entrant is attempting to undermine the legitimate operation of the Contest by cheating, hacking, deception, or other unfair playing practices (including the use of automated quick entry programs) or intending to annoy, abuse, threaten or harass any other entrants or Sponsor’s representatives. All federal, provincial, and local laws, rules, and regulations apply.

WINNERS LIST: For written confirmation of the prizewinner’s name and city of residence, send your request and a self-addressed stamped envelope with correct postage to the Sponsor.
